REQUIREMENTS

1. Fill in the tourist visa form or work visa form.
2. A valid passport for at least 6 months (no blank passport).
3. Photocopy of FULL COLOR Passport on all passport pages. (A4 size and not back and forth)
4. 2 (two) pieces of 3.5 x 4.5 white background photograph with 70% zoom.
5. Employment certificate / company sponsorship for employees.
• If the position of Manager, Director, and Commissioner please attach TIN and Trade Business License of the Company.
• If you have your own business sponsorship letter typed on white paper and stamped, do not forget to attach NPWP and SIUP.
• If sponsored by a family (child / husband / parent) please attach a document that explains the relationship, for example birth certificates / family cards / marriage certificates, etc.
6. Financial proof of the last 3 months using current account or savings book.
7. Photocopy of family card and photocopy of ID Card (KTP).
8. If the name listed in the passport is different then attach a photocopy of the letter change name.
9. Photocopy of marriage certificate.
10. If the child travels and is still in school, then attach:
• Photocopy of student card
• Photocopy of birth certificate
11. Print out a ticket reservation.
12. Hotel confirmation while in New Zealand.
13. The form has been filled in completely and signed by the applicant. For children who do not have an ID card(KTP), the parents have to sign the form.

NOTES

1. DOCUMENTS CANNOT BE STAPLED, both forms and supporting documents from visa applicants
2. The original passport will still be taken at the time of applying for the visa to be shown at the counter and will be returned immediately.

• The processing time for making a visa starts from the time all document files are submitted to the Service Transfer Office or Embassy

• The original passport is still brought when applying for a visa to be shown at the counter and will be returned immediately
